The Hudson County Board of Commissioners has approved an application and acceptance of funds, if awarded, from the Centers for Disease Control and Prevention for the Advancing Health Equity for Priority Populations with, or at Risk for, Diabetes Grant Program.

The county has applied for $5,500,000 in grant money from the Diabetes Grant Program.

The term of the grant will be from May 30, 2023 through May 29, 2028.

The Hudson County Department of Health and Human Services, Office on Health Equity, proposes to support residents with or at risk for developing Type 2 Diabetes, by addressing systematic obstacles to accessing health care and other services within Hudson County.

The grant will be used to decrease the risk for type 2 diabetes among adults with pre-diabetes and improve self-care practices, quality of care, and early detection of complications among people with diabetes.
