HOBOKEN, N.J. — The Hoboken City Council has been called into a special virtual meeting for Tuesday, Oct. 27, at 5:00 p.m. to go into closed session on labor relations and vote on a $1,274,028 contract amendment with Judi Rx (formerly Capital Rx) to move 523 Medicare-eligible city retirees and their dependents into a level-funded prescription drug plan starting Jan. 1, 2027, according to the newly posted agenda packet.
The special meeting notice, signed Friday, Oct. 23, by Council President Ruben Ramos Jr. with an attached call letter from Mayor Emily B. Jabbour, lists only two substantive items: a closed executive session with Corporation Counsel Diana Jeffrey to receive legal advice on labor relations, followed by Resolution A1, sponsored by Councilman Steve Firestone, approving the 2027 Employer Group Waiver Plan (EGWP) with Judi Rx Retire.
$1.01M projected savings — and an uncapped tail risk above $491 per member
According to Resolution A1, switching from Hoboken’s current Medicare Part D retiree pharmacy setup to the Judi Rx EGWP is projected to save the city approximately $1,013,600 a year while maintaining existing coverage levels. Chief Financial Officer Chris Baldwin certified up to $1,274,028 from the city’s Self Insurance Fund (Account T-21-20-700-023) for calendar year 2027, based on an estimated monthly contribution of $203 per member per month (PMPM) across 523 enrolled retirees and dependents.
However, the council resolution and the attached 12-page Judi Rx Retire proposal (dated Oct. 14, 2026) describe the city’s risk exposure differently:
- What the resolution text says: Clauses 3 and 7 of Resolution A1 state that Judi Rx submitted a proposal for a “fully insured Employer Group Waiver Plan” that would transition Hoboken “from variable retiree pharmacy costs to a predictable, fully insured premium structure, reducing exposure to pharmacy claim volatility and improving budget certainty.”
- What the attached Judi Rx contract proposal says: On , under the heading Important, Judi Rx’s proposal explicitly warns: “This proposal describes a level-funded arrangement with limited aggregate stop-loss protection. It should not be presented as a fully insured premium product, and it does not eliminate the plan sponsor’s claim liability above the stop-loss maximum.”
- Where the stop-loss protection ends: Under the quote terms on pages 19 and 21 of the packet, aggregate stop-loss insurance through Tokio Marine (subject to final underwriting) attaches when eligible net pharmacy claims exceed $173 PMPM (119% of expected net claims) and covers claims up to a maximum of $491 PMPM (337% of expected net claims). Above $491 PMPM, 100% of eligible net claims revert to the City of Hoboken.
How the $1,274,028 annual price breaks down
Hoboken originally extended its pharmacy contract with Capital Rx (now operating as Judi Rx, part of Judi Health) on July 10, 2024. Under the 2027 level-funded EGWP quote for 523 Medicare-eligible retirees, each monthly payment combines expected net drug claims, Judi Rx’s administration fee, and the Tokio Marine stop-loss premium:
| Cost Component | Per Member / Month (PMPM) | Estimated Monthly Total (523 Retirees) | Estimated Annual Total (12 Months) |
|---|---|---|---|
| Expected gross pharmacy claims | $647 | $338,381 | $4,060,572 |
| Less expected rebates, Medicare Part D subsidies & manufacturer offsets | ($501) | ($262,258) | ($3,147,096) |
| Expected net claim funding (City funded) | $146 | $76,123 | $913,472 |
| Judi Rx Retire administrative fee | $27 | $14,121 | $169,452 |
| Aggregate stop-loss premium / risk charge (Tokio Marine) | $30 | $15,925 | $191,104 |
| Total Level-Funded Contribution | $203 | $106,169 | $1,274,028 |
If actual net pharmacy claims come in below the $146 PMPM projection, Judi Rx notes that unused claim funding may be returned or credited to the city only if expressly provided in the final executed contract terms.
Labor unions, closed session, and retiree copays
Before voting on the Judi Rx resolution, the council is scheduled to convene in closed session at 5:00 p.m. with Corporation Counsel Diana Jeffrey under N.J.S.A. 10:4-12(b)(7) “to discuss legal matters pertaining to labor relations.” Clause 5 of Resolution A1 states that the city has determined the proposed EGWP “constitutes a change in the method of funding and administering retiree prescription drug benefits, and not a reduction in the level of benefits provided under applicable collective negotiations agreements.”
Under the proposed 2027 plan design (packet p. 18), retirees would use the five-tier Prime Premier Formulary with an annual out-of-pocket maximum of $2,400 (after which retirees pay $0 for covered Part D drugs) and a $35 monthly cap on covered insulin ($105 for a 90-day supply). Standard 30-day retail copays are listed at $10 for preferred and standard generics, $30 for preferred brand drugs, $50 for non-preferred brand drugs, and 25% coinsurance for specialty medications.
Tuesday’s special meeting begins at 5:00 p.m. via Zoom (Webinar ID: 880 4743 2123; Passcode: 744225) and on the city’s IQM2 meeting portal, with public comments limited to five minutes per speaker.
